Kuroshima's people live in five small hamlets spread across the island, and Higashisuji in the middle holds the post office and the observation tower.

Hori sits by the port, Iko on the north-east coast, Nakamoto and Miyazato in the west, and Higashisuji in the centre. Higashisuji has the post office and the village shop, Tama Shoten, and the observation tower stands beside its school. Miyazato holds the national park visitor centre and the Kuroshima Research Institute, which studies sea turtles and coral.
Cattle shape everything here, with more than 3,000 head against about 220 people, and the island holds its cattle festival each February. Heart Land café sits just up from the port and opens all day. Soba Cafe Undouya serves lunch from 11:00 to 14:00, and iconoma near Iko pier opens in the afternoon.
A public rideshare run by Route Kuroshima carries visitors between the port and the hamlets by phone booking, and Kuroshima Rental Cycle rents bikes by the hour near the port.
